Service Overview

All clients referred to the service are Individualised assessment by cardiac nurse and cardiac physiotherapist. Post assessment clients are offered a cardiac rehabilitation service that meets their individual requirements and agreed by client and clinician ie centre based, home based program All client are offered a follow up assessment after an agreed length of time usually 6 weeks post completion of centre based program

Eligible Patients

Diagnosis

  • Acute coronary syndrome: acute myocardial infarction (heart attack) & stable or unstable angina (chest pain)
  • Coronary heart disease
  • Heart failure
  • Cardiomyopathies

    • Procedures

  • Re-vascularisation - coronary angioplasty or stenting
  • Coronary artery bypass graft (CABG) surgery
  • Valve surgery
  • Cardiac Transplant
  • Defibrillator or pacemaker implantation
  • Cardiac surgery: AAA repair, congenital, LVADS, Arrhythmia Surgery or combination
