Monday to Friday by Chronic Care CNC

Outpatient (Cardiac Rehabilitation) What is this?

7 to 8am Tuesdays and Fridays over 12 weeks for working/returning to work clients. Gym only 8 and 9.30am Tuesdays and Fridays over 8 weeks includes gym and education 10 to 1115am Wednesday over 10 weeks. Gym only 'Healthy Heart' seminar day each month 0800 to 1500

Maintenance (Cardiac Rehabilitation) What is this?

Minimal hospital based maintenance available but a range of community based programs are offered

Heart Failure Management  What is this?

This is managed by the SHALT team at Sutherland Hospital

Eligible Patients

Diagnosis

  • Acute coronary syndrome: acute myocardial infarction (heart attack) & stable or unstable angina (chest pain)
  • Coronary heart disease
  • Cardiomyopathies
  • Arrhythmia (abnormal heart rhythm) - Atrial fibrillation, atrial flutter, supraventricular tachycardia (SVT)
  • Adult congenital heart disease, pulmonary hypertension, peripheral vascular disease
  • Cardiac Surgery

    • Procedures

  • Re-vascularisation - coronary angioplasty or stenting
  • Coronary artery bypass graft (CABG) surgery
  • Valve surgery
  • Cardiac Transplant
  • Defibrillator or pacemaker implantation
  • Cardiac surgery: AAA repair, congenital, LVADS, Arrhythmia Surgery or combination
