Service Overview

Flexible Model of care centred/home/phone 8 week rolling program, Wednesdays 10am - 12.30pm. education and exercise sessions Mondays 10am to 1100am exercise sessions, Comments:multidisciplinary involvement, individual assessment with cardiac rehab nurse and exercise physiologist weekly. Carers and significant others encouraged to attend. Patient Eligibility Criteria - ACS, angioplasty, stenting, bypass, valve surgery, heart failure

Services

Inpatient (Cardiac Rehabilitation) What is this?

Monday to Friday. Education to all STEMI, NSTEMI, PCI, High Risk Factors and patients referred for cardiac surgery. Access to multidisciplinary allied health teams.

Outpatient (Cardiac Rehabilitation) What is this?

Wednesdays 10am - 12.30pm. education and exercise sessions Mondays 10am to 1100am exercise sessions,

Maintenance (Cardiac Rehabilitation) What is this?

Thursday 1200- 115pm school terms only Patients must have completed the Outpatient program (offered for up to six months)

Heart Failure Management  What is this?

Heart failure patients receive education on the benefits of exercise, managing of their condition (diet, sodium restrictions, fluid management, action plans) and access to a staged exercise rehabilitation program.

Eligible Patients

Diagnosis

  • Acute coronary syndrome: acute myocardial infarction (heart attack) & stable or unstable angina (chest pain)
  • Coronary heart disease
  • Heart failure
  • Cardiomyopathies
  • Arrhythmia (abnormal heart rhythm) - Atrial fibrillation, atrial flutter, supraventricular tachycardia (SVT)
  • Cardiac Risk Factor
  • Procedures

  • Re-vascularisation - coronary angioplasty or stenting
  • Coronary artery bypass graft (CABG) surgery
  • Valve surgery
  • Cardiac Transplant
  • Defibrillator or pacemaker implantation
