Service Overview
The Blacktown Heart Failure service provides support and education to people with a diagnosis of Heart failure through echocardiographic assessment. Inpatient education, outpatient clinic follow-up, phone support & home visits are offered. Patients are supported by a Nurse Practitioner, Clinical Nurse Consultant and Registered Nurse.

Inpatient (Cardiac Rehabilitation) What is this?
The heart failure service offers rapid assessment, stabilisation and/or maintenance clinic support. We offer education on the disease process, symptom awareness and self-management strategies. Referral to a staged exercise program, health coaching and care facilitation is available.
