Phase II Cardiac Rehab (Rural)

Services

Inpatient (Cardiac Rehabilitation) What is this?

Home visit by Community Health Nurse, usually every 3 weeks for approx. 6 months or until client is confident they can maintain a healthy lifestyle and has reached goals set at start of rehab. Also includes follow-up phone calls.

Outpatient (Cardiac Rehabilitation) What is this?

Home visits monthly to perform health check and discuss goals/strategies to prevent relapse of ill health.

Maintenance (Cardiac Rehabilitation) What is this?

Heart Failure Management  What is this?

Eligible Patients

Diagnosis

  • Acute coronary syndrome: acute myocardial infarction (heart attack) & stable or unstable angina (chest pain)
  • Coronary heart disease
  • Heart failure
  • Arrhythmia (abnormal heart rhythm) - Atrial fibrillation, atrial flutter, supraventricular tachycardia (SVT)
  • Adult congenital heart disease, pulmonary hypertension, peripheral vascular disease
  • Procedures

  • Re-vascularisation - coronary angioplasty or stenting
  • Coronary artery bypass graft (CABG) surgery
  • Valve surgery
  • Defibrillator or pacemaker implantation
  • Cardiac surgery: AAA repair, congenital, LVADS, Arrhythmia Surgery or combination
