Home visit by Community Health Nurse, usually every 3 weeks for approx. 6 months or until client is confident they can maintain a healthy lifestyle and has reached goals set at start of rehab. Also includes follow-up phone calls.

Home visits monthly to perform health check and discuss goals/strategies to prevent relapse of ill health.

Acute coronary syndrome: acute myocardial infarction (heart attack) & stable or unstable angina (chest pain)

Coronary heart disease

Heart failure

Arrhythmia (abnormal heart rhythm) - Atrial fibrillation, atrial flutter, supraventricular tachycardia (SVT)

Adult congenital heart disease, pulmonary hypertension, peripheral vascular disease

Re-vascularisation - coronary angioplasty or stenting

Coronary artery bypass graft (CABG) surgery

Valve surgery

Defibrillator or pacemaker implantation

Cardiac surgery: AAA repair, congenital, LVADS, Arrhythmia Surgery or combination
