Service Overview
The Cardiac Rehabilitaion and Heart Failure Service is a multidisciplinary community based program located at the Robina Health Precinct and Helensvale Community Health Centre. It aims to improve the health status and quality of life for people living with Cardiac disease and heart failure. The Cardiac Rehabilitation and Heart Failure Service has a strong focus on health education and self management skills, physical exercise, risk factor modification.
Services
Inpatient (Cardiac Rehabilitation) What is this?
Monday to Friday. Education to all STEMI, NSTEMI, PCI, High Risk Factors and patients referred for cardiac surgery. Access to multidisciplinary allied health teams.
Outpatient (Cardiac Rehabilitation) What is this?
12 session program, Tuesday and Thursdays 7.00am; 8.45am; 11.15am.
10am - 11am Education Tuesday and Thursday.
Fast Track Education once a month
Maintenance (Cardiac Rehabilitation) What is this?
A dedicated multidisciplinary program of care for patients with chronic heart failure providing face to face patient education with a strong focus on developing self management skills, medication titration, fluid management, along with intensive case management.

Heart Failure Management What is this?
A dedicated program of care that utilises the expertise of the multidisciplinary teams. Services include, fluid management and healthy eating plans, tailored exercise program, mindfulness, fatigue management, future planning and advance care plans and medication education.
