Service Overview

Cardiac rehabilitation program offered to all clients with cardiac diagnosis or high risk for cardiac event. Individual assessment with cardiac nurse followed by 6 week program. Access to individual dietician appointment if required.

Inpatient (Cardiac Rehabilitation) What is this?

Inpatient education provided for patients with cardiac diagnosis. Referral to outpatient cardiac rehab program post discharge

Outpatient (Cardiac Rehabilitation) What is this?

6 session program over 6 weeks. Fridays 8 - 9.30am Aerobic and resistance exercise session followed by education delivered by multidisciplinary team .

Maintenance (Cardiac Rehabilitation) What is this?

Heart failure education provided in individual appointments. Access to general cardiac rehab program if stable.

Heart Failure Management  What is this?

Eligible Patients

Diagnosis

  • Acute coronary syndrome: acute myocardial infarction (heart attack) & stable or unstable angina (chest pain)
  • Coronary heart disease
  • Heart failure
  • Cardiomyopathies
  • Arrhythmia (abnormal heart rhythm) - Atrial fibrillation, atrial flutter, supraventricular tachycardia (SVT)
  • Adult congenital heart disease, pulmonary hypertension, peripheral vascular disease

    • Procedures

  • Re-vascularisation - coronary angioplasty or stenting
  • Coronary artery bypass graft (CABG) surgery
  • Valve surgery
  • Defibrillator or pacemaker implantation
  • Cardiac surgery: AAA repair, congenital, LVADS, Arrhythmia Surgery or combination
