Individual and group education and exercise. Inpatient and outpatient service. Post revascularisation procedures (CABG, PCI), valve surgery, ACS (STEMI, NSTEMI, angina, high risk factor profile). Secondary service at Integrated Care Centre - ACS and PCI Accept referrals from Private hospitals and GP's

Inpatient (Cardiac Rehabilitation) What is this?

Pre-discharge education for revascularisation and ACS patients Monday to Friday.

Outpatient (Cardiac Rehabilitation) What is this?

8 week program including individual comprehensive pre- and post program assessment. 6 sessions education and supervised exercise. Tuesday and Wednesday 10-12.30 Thursday 1300-1530 (RHH); Tuesday 9.30-1200 (ICC)

Maintenance (Cardiac Rehabilitation) What is this?

Heart Failure Management  What is this?

Heart failure patients receive education on the benefits of exercise, managing of their condition (diet, sodium restrictions, fluid management, action plans). Referrals to cardiopulmonary rehabilitation program available.

Eligible Patients

Diagnosis

  • Acute coronary syndrome: acute myocardial infarction (heart attack) & stable or unstable angina (chest pain)
  • Coronary heart disease
  • Heart failure
  • Cardiomyopathies
  • Arrhythmia (abnormal heart rhythm) - Atrial fibrillation, atrial flutter, supraventricular tachycardia (SVT)
  • Procedures

  • Re-vascularisation - coronary angioplasty or stenting
  • Coronary artery bypass graft (CABG) surgery
  • Valve surgery
  • Defibrillator or pacemaker implantation
  • Cardiac surgery: AAA repair, congenital, LVADS, Arrhythmia Surgery or combination
