Service Overview

The Outpatient Service provides education, prescribed exercise, counselling, motivation and skill building for patients to modify aspects of their behaviour that are contributing to their cardiovascular risk , to assist them to better self manage their condition and improve their clinical outcomes and well-being. The program guides the patient through their recovery, resuming their usual activities and psychological adjustment to their cardiac event and diagnosis.

Services

Inpatient (Cardiac Rehabilitation) What is this?

Education to all STEMI, NSTEMI, AF, PCI, CP , high risk factors, arrhythmias and patients referred for cardiac surgery. Access to multidisciplinary allied health team.

Outpatient (Cardiac Rehabilitation) What is this?

An individualised program with length dependent on needs. Individual and group education and counselling. Individualised exercise and training. Patients include those with a cardiac diagnosis and those at high risk of heart disease. Sessions of exercise and education Tuesdays and Thursdays.

Maintenance (Cardiac Rehabilitation) What is this?

Ongoing at 6 months post outpatient program then 12 months. Individual assessment of risk factors, symptoms, psychosocial assessment, counselling on self management. Exercise classes Mondays and Fridays. Patients must have completed the Outpatient program.

Heart Failure Management  What is this?

Ongoing program, inpatient education on self management, home visits after discharge, telephone support and a weekly exercise program. South

Eligible Patients

Diagnosis

  • Acute coronary syndrome: acute myocardial infarction (heart attack) & stable or unstable angina (chest pain)
  • Coronary heart disease
  • Heart failure
  • Cardiomyopathies
  • Arrhythmia (abnormal heart rhythm) - Atrial fibrillation, atrial flutter, supraventricular tachycardia (SVT)
  • Adult congenital heart disease, pulmonary hypertension, peripheral vascular disease

  • Re-vascularisation - coronary angioplasty or stenting
  • Coronary artery bypass graft (CABG) surgery
  • Valve surgery
  • Cardiac Transplant
  • Defibrillator or pacemaker implantation
  • Cardiac surgery: AAA repair, congenital, LVADS, Arrhythmia Surgery or combination
